Social Prescribing

Our Social Prescribing program supports patients referred by healthcare providers across West Ottawa. Through this initiative, our Social Prescriber Counsellor offers personalized, one-on-one support to help individuals navigate social, emotional, and practical challenges that influence their health.

By connecting clients with local community services, this program provides a holistic, non-medical approach that addresses the social determinants of health—including housing, income, education, social support, and community connection.

Research shows that social prescribing can:

  • Improve psychological well-being
  • Reduce anxiety
  • Increase perceived quality of life
  • Lower demand on healthcare services, including primary care visits, ER usage, and secondary care referrals

By linking people with existing community resources, our program delivers meaningful support at a relatively low cost while easing pressure on the healthcare system.
